Uroplakin III is a glycoprotein (47 kD) of the asymmetrical unit. Uroplakin III stains the urothelial surface membrane in paraffin sections of human renal pelvis, ureter, bladder, and urethra. This marker, while of only intermediate sensitivity, is highly specific to uroepithelial tumors. About 60% of human transitional cell carcinomas (including metastases) maintain focal (sometimes very limited) expression of uroplakin III. Loss of UP III expression has been associated with high-grade, muscle-invasive cancer, lymphovascular invasion. Studies have also shown Uroplakin III expression was restricted to a subset of p63-negative carcinoma cells, including anaplastic carcinoma cells; and has shown that, Uroplakin III is immunoreactive in secondary vulvar Paget disease of urothelial origin.
