Studies have shown that Glypican-3 (GPC3) protein is a very sensitive marker for hepatocellular carcinomas (HCC), but is undetectable in normal liver and benign hepatic lesions, including dysplastic and cirrhotic nodules. In-house TMA based studies have shown that GPC3 is positive in 90.4% of hepatocellular carcinoma cases and negative in 100% of cholangiocellular carcinoma, normal liver and hyperplasia cases. Several published studies show GPC3 staining 78%-84% of HCC, and when compared with immunohistochemistry of hepatocyte specific antigen (HSA) and alphafetoprotein, GPC3-immunohistochemistry was significantly more specific and sensitive for hepatocellular carcinomas. In liver disease, CK19 is of prognostic value in HCC and may be used in distinguishing cholangiocellular carcinoma (CK19+) from HCC (CK19-). It can also be used to highlight native ductules in the liver and helps separate conditions such as focal nodular hyperplasia (CK19+) from hepatic adenoma (CK19-). Studies also show CK19 expression as an independent prognostic factor for HCC with lymph node metastasis. It is of clinical significance for treatment modalities to differentiate HCC with intrahepatic cholangiocellular carcinoma-like differentiation (CK19+) from HCC with a higher T-classification (CK19-).

The combination of the GPC3 and CK19 multiplex stain can help distinguish hepatocellular carcinomas vs cholangiocellular carcininomas vs. metastatic tumors of the liver. Furthermore, this Multiplex IHC may assist in distinguishing nodular hyperplasia from hepatic adenoma.
